These families play an essential role in providing daily care and support for their disabled loved ones. However, the support they receive is limited, and they have significant additional costs to bear. This is why Makivvik created Lilly\u2019s fund, to provide additional support to these families.<\/p>\n\n\n\n<p>Lilly\u2019s Fund was named in recognition of the advocacy work of Lilly Mina, from Inukjuak. She tirelessly sought to raise awareness of the challenges faced by Nunavik Inuit families who were caring for a severely disabled family member.<\/p>\n\n\n\n<p><strong>What kind of support is available through Lilly\u2019s Fund?<\/strong><\/p>\n\n\n\n<p>A beneficiary with a severe and permanent physical or intellectual disability, or his\/her parent or caregiver, may be eligible to receive one of the following amounts based on the program\u2019s criteria:<\/p>\n\n\n\n<p>\u2013 $12,000 per person per year, in 2025-2026, if the disabled person lives at home<br>\u2013 $6,000 per person per year if the disabled person lives in a residential facility<\/p>\n\n\n\n<p><strong>Who can apply for financial support from Lilly\u2019s fund?<\/strong><\/p>\n\n\n\n<p>Lilly\u2019s fund was designed to help Nunavik Inuit who are living with severe and permanent disabilities, as well as their parents or guardians if they are minors.<\/p>\n\n\n\n<p>The following criteria determine eligibility for the program:<\/p>\n\n\n\n<p>\u2013 Be a beneficiary of the <em>JBNQA<\/em>.<br>\u2013 Living with a severe and permanent physical or intellectual disability, as established by a healthcare professional.<br>\u2013 Being 64 years old or younger.<\/p>\n\n\n\n<p><strong>Do I need to use a form for my application?<\/strong><\/p>\n\n\n\n<p>Yes, the Lilly\u2019s Fund Program Application Form can be downloaded from the Makivvik website https:\/\/www. makivvik.ca\/lillys-fund-support-program\/<\/p>\n\n\n\n<p><strong>How do I apply for financial support from Lilly\u2019s Fund?<\/strong><\/p>\n\n\n\n<p>To apply for financial support from Lilly\u2019s Fund, please send a completed and signed copy of the Lilly\u2019s Fund Program Application Form to the following email address: Lillysfund@makivvik.ca<\/p>\n\n\n\n<p>There are two ways to assess a beneficiary\u2019s eligibility for the program:<\/p>\n\n\n\n<p>\u2013 Either the beneficiary is eligible for the Canada Disability Tax Credit, in which case they simply need to attach proof of this eligibility (letter, etc.) to the Lilly\u2019s Fund Program Application Form.<\/p>\n\n\n\n<p>\u2013 Either the beneficiary does not know if they are eligible for the Canada Disability Tax Credit, in which case an assessment by a healthcare professional is required. This assessment allows the healthcare professional to sign the Lilly\u2019s Fund Program application form.<\/p>\n\n\n\n<p><strong>How can I be evaluated by a healthcare professional?<\/strong><\/p>\n\n\n\n<p>A healthcare professional will carry out the assessment through the \u201cAccess Point \u2013 Functional Limitation Programs\u201d of the Nunavik Regional Board of Health and Social Services. A professional at your local CLSC can carry out this assessment and complete the Lilly\u2019s Fund Program application form following it.<\/p>\n\n\n\n<p><strong>What happens once the form has been submitted?<\/strong><\/p>\n\n\n\n<p>After submitting the form to Lillysfund@makivvik.ca with proof of eligibility for the Canada Disability Tax Credit or following an assessment by a healthcare professional, you will receive written confirmation via email and\/or mail.<\/p>\n\n\n\n<p>Your application will then be reviewed to ensure that it is complete. You will be contacted if any information is missing.<\/p>\n\n\n\n<p>If you meet the program criteria and are eligible, you will receive a confirmation letter containing important information.<br><br><strong>Does the application have to be renewed every year?<\/strong><\/p>\n\n\n\n<p>A renewal form will be sent to you each year to update your file.<\/p>\n\n\n\n<p><strong>Can I get help filling out my application?<\/strong><\/p>\n\n\n\n<p>If you need help filling out the form, please contact Makivvik. The Lilly\u2019s Fund webpage will be kept up to date so that you can find the contact details of the person who can help you.<\/p>\n\n\n\n<p><strong>Can I receive Direct Deposit?<\/strong><\/p>\n\n\n\n<p>Yes.
